What is Heel Spur? Epin calcanei (Calcaneal spur / Heel spur) is a bony protrusion that develops on the underside of the heel bone. It is the first reason that comes to mind in heel area pain. It is most often associated with plantar fasciitis and is one of the common causes of heel pain. Pain may also occur due to edema of the heel fat pad without a bony prominence. Reasons • Recurrent microtraumas on the plantar fascia and heel • Standing for long periods of time, walking on hard surfaces • Obesity, sudden weight gain • Flat feet or high arch of the foot • Inappropriate shoe use • Rheumatic diseases (e.g. spondyloarthritis) What are the symptoms? • Stinging pain in the heel, especially when taking the first step in the morning or when walking after sitting for a long time • Pain that may decrease with movement during the day and increase in the evening • Not every bone spur may cause pain; pain is often caused by accompanying plantar fasciitis How is the diagnosis made? • Clinical examination examines the location and spread of pain. • Visibility of bony prominence with direct radiography • MRI is rarely required Note: The presence of epine in the graph does not always cause pain. What is done in treatment? (Stepwise treatment is applied) first choice: • Activity regulation, weight control • Heel cushion / insoles, suitable shoes • Stretching exercises (plantar fascia, Achilles) • medications • Physiotherapy, night splint • ESWT (shock wave therapy) Interventional: • Corticosteroid injection (in selected cases)
